Hi Doctor, I’ve had long-standing mental health issues since childhood. I form intense attachments (mostly toward males), idealize people, become obsessive (excessive texting/calling/stalking), and feel extreme distress when they leave. I struggle with emotional instability, anger, identity and goal changes, compulsive behaviors (including excessive masturbation and repetitive “new life” fantasies), and difficulty adjusting to change. I sometimes feel paranoid, have thoughts of harming others (without acting), and occasional suicidal thoughts. Past clinicians have suggested OCD, BPD, or bipolar disorder, and I’m confused about the diagnosis and whether I should see a psychiatrist or psychologist first.

Hey, from all that you have narrated, it seems obvious that you have been and still are going through a lot of distress. I can understand your pain.
It seems that you are carrying a huge amount of childhood baggage. As I was going through the symptoms that you've mentioned, I stopped and pondered on the occasional suicidal thoughts.
My next question would be, have you ever made a plan regarding bringing this to action? Have you ever attempted to do so? Have you ever discussed this with someone you trust?
If the answer to any of these questions is Yes, then I'll advise you to seek one-on-one professional help. In case your answer to all these is No, then you need to perform the technique named DBT. All techniques are out there on YouTube, but being attached with a licensed clinical psychologist and performing under supervision is much safer. You will remain committed and accountable.
In case there is sleep disturbance or any change in your appetite, medications may be required. For that, you'll require to visit a psychiatrist.

DBT is a very effective therapy. Like I mentioned earlier, it is always better to remain under supervision because it keeps you committed and accountable. Being accountable to someone is a total commitment in itself.
In my opiniin, it is best to attach yourself to a clinical psychologist, whom you can trust and feel safe with.

The symptoms you mentioned are of BPD and it can sometimes present with episodic states of psychosis or any other psychistric illness...you need to see a psychiatrist first and then a psychologist for management of both personality and psychiatric illness
