An HCV PCR test costs Rs 4,900 at Marham Lab for the qualitative test, down from Rs 7,000, and Rs 6,650 for the quantitative viral load. CDC explains that an antibody test shows whether you have ever been infected, while a nucleic acid test for HCV RNA shows whether the virus is still in your blood. A positive anti HCV result therefore needs a PCR before anything is decided.

What is the HCV RNA by Real-Time PCR (Qualitative) test?

Hepatitis C is a liver infection caused by the hepatitis C virus. CDC states that many people do not have symptoms, so testing is the only way to know if you have it, and that left untreated chronic hepatitis C can cause serious health problems including liver damage, cirrhosis, liver cancer and even death. An HCV PCR test looks for the virus itself in your blood. At Marham Lab the qualitative test costs Rs 4,900, reduced from Rs 7,000.

Anti HCV and HCV PCR Are Not the Same Test

This is the single most important thing on this page, and it is the reason most people are reading it.

CDC explains that antibody tests are used to find out if you have ever been infected with HCV, and that a positive or reactive result may not mean you currently have hepatitis C. It goes further: once infected, you will always have antibodies in your blood. That stays true whether you cleared the virus on your own, were cured by treatment, or still carry it.

So a positive [anti HCV result](/labs/tests/hepatitis-c-test-price) is not a diagnosis of current infection. It is a signal that a second test is needed.

That second test is the one on this page. CDC calls it a nucleic acid test, or NAT, for HCV RNA, and says it looks for the presence of the virus in your blood. CDC notes that when an antibody test is reactive the laboratory will normally run this test automatically on the same sample.

ResultWhat CDC says it means
Anti HCV non reactiveYou are not currently infected. No follow up PCR is needed
Anti HCV reactive, PCR negativeYou were previously infected and the virus is no longer in your body
Anti HCV reactive, PCR positiveThis confirms you are currently infected with HCV

A great many people in Pakistan are told they are hepatitis C positive on the strength of an antibody test alone. Some of them cleared the infection years ago. The PCR is what settles it.

When to Skip the Antibody Test Entirely

CDC gives one clear exception. If you might have been exposed to HCV within the past 6 months, your doctor should order a NAT for HCV RNA rather than an HCV antibody test.

The reason is timing. Antibodies take time to appear, so a recent exposure can produce a negative antibody result while the virus is already present. The PCR detects the virus directly and does not have to wait for your immune system to respond.

Tell your doctor if there was a specific recent exposure, such as a needle stick, a blood transfusion, a dental or surgical procedure, or a shared razor or syringe.

Qualitative or Quantitative: Which One to Book

Marham Lab lists two PCR tests, and they answer different questions.

TestWhat it answersMarham LabChughtai Lab
HCV RNA by PCR, qualitativeIs the virus in my blood right now, yes or noRs 4,900Rs 6,400
HCV RNA by PCR, quantitative (viral load)How much virus is in my bloodRs 6,650Rs 8,800
HCV genotyping by PCRWhich strain of the virus you carryNot currently listedRs 11,800

The qualitative test is the one that follows a reactive antibody result, because the question at that point is simply whether the infection is active. The quantitative viral load is generally used around treatment, to measure the starting level and to check the response afterwards. Genotyping identifies the strain.

Book whichever your doctor wrote. If the prescription says only HCV PCR, the qualitative test is the usual meaning and it is the cheaper of the two.

Who Should Be Tested

CDC recommends hepatitis C screening for all adults 18 and older at least once in their lifetime, and for pregnant women during each pregnancy.

CDC recommends at least one time testing for these groups.

  • People who currently inject drugs, or ever did so once or long ago
  • People who have HIV
  • People with abnormal liver tests, liver disease, or who are on haemodialysis
  • People who received blood or organ donations before July 1992
  • People who received clotting factor concentrate before 1987
  • Health care, emergency medical and public safety staff after needle sticks, sharps or mucosal exposure to HCV positive blood
  • Infants and children born to people with known HCV infection

CDC recommends periodic testing for people with ongoing risk factors, people who have ever received maintenance haemodialysis, and any person who requests hepatitis C testing.

Pakistan carries one of the heaviest hepatitis C burdens in the world, and reused syringes and unsterilised dental and barber equipment are recognised routes here. If you have had an injection, a dental procedure or a shave with a shared blade in a setting you are unsure about, testing once is reasonable.

The Result That Matters Most

CDC is direct about what happens after a confirmed infection: there are treatments that can cure most people with hepatitis C within 8 to 12 weeks.

That is the reason this test is worth the money. Hepatitis C is now a curable infection for most people, and the tablets that do it are taken at home. What stands between a person and that outcome is usually a diagnosis nobody completed, because an antibody test came back reactive and the PCR was never done.

If your PCR is positive, take the report to a gastroenterologist or hepatologist rather than waiting for symptoms.

How to Prepare

No fasting is needed and no preparation is required.

Bring your previous anti HCV report if you have one, and tell the phlebotomist whether you are being tested after a reactive antibody result, before treatment, or after finishing treatment. Those three situations are read differently.

If you have already started antiviral treatment, tell your doctor. Treatment lowers the viral load, which changes how the result should be interpreted.

What Happens During the Test

A trained phlebotomist takes a small blood sample from a vein in your arm using a fine needle. It takes under five minutes, and a brief sting or slight bruising afterwards is normal. In Lahore the sample can be collected at your home at no extra cost.

The report takes 3 to 4 days, because PCR requires the sample to be processed in batches on specialised equipment rather than on a routine analyser.

Final Thoughts

If you remember one thing, make it this. An antibody test tells you that you met this virus at some point in your life. A PCR tells you whether it is still there. Those are different facts, and only the second one decides whether you need treatment. Book the PCR after any reactive antibody result, book it directly if the possible exposure was within the last six months, and if it comes back positive, see a liver specialist promptly, because most people are cured in 8 to 12 weeks.

Frequently asked questions

The qualitative HCV RNA by PCR test costs Rs 4,900 at Marham Lab, reduced from Rs 7,000, with free home sample collection in Lahore. The same test at Chughtai Lab is Rs 6,400. The quantitative viral load is Rs 6,650 at Marham Lab and Rs 8,800 at Chughtai Lab. HCV genotyping by PCR is Rs 11,800 at Chughtai Lab. The report takes 3 to 4 days.

Not necessarily, and this is the most common misunderstanding about hepatitis C testing. CDC states that a reactive antibody result means you have been infected at some point in your life, and that once infected you will always have antibodies in your blood, whether you cleared the virus, were cured, or still carry it. Only a nucleic acid test for HCV RNA, the PCR on this page, shows whether the virus is in your blood now. A reactive antibody with a negative PCR means the infection is gone.

Whichever your doctor wrote. The qualitative test answers whether the virus is present at all and is the one that normally follows a reactive antibody result. It costs Rs 4,900 at Marham Lab. The quantitative viral load measures how much virus is present and is generally used around treatment, at Rs 6,650. If the prescription says only HCV PCR, the qualitative test is the usual meaning and the cheaper option.

CDC is specific on this point. If you might have been exposed to HCV within the past 6 months, your doctor should order a nucleic acid test for HCV RNA rather than an antibody test. Antibodies take time to develop, so a recent infection can produce a negative antibody result while the virus is already present. The PCR detects the virus directly. Tell your doctor about the exposure, such as a needle stick, transfusion, or a dental or surgical procedure.

For most people, yes. CDC states that there are treatments that can cure most people with hepatitis C within 8 to 12 weeks. That is why completing the diagnosis matters. CDC also notes that left untreated, chronic hepatitis C can cause liver damage, cirrhosis, liver cancer and death, and that many people have no symptoms at all. If your PCR is positive, take the report to a gastroenterologist or hepatologist rather than waiting for symptoms to appear.
