What is the LFTs (Liver Function Tests) test?
An LFT is a single blood test that measures how well your liver is working and whether it has been damaged. One sample gives your doctor a set of numbers: bilirubin, the enzymes ALT, AST, ALP and GGT, and the proteins albumin and total protein.
Your liver does hundreds of jobs quietly. It clears waste, makes proteins, processes medicines and handles fat. When it is under strain, the first sign is usually not pain. It is a number moving on this report.
LFTs, Liver Function Test and LFT: The Same Test
If your slip says LFT, LFTs, liver function test, liver profile or liver panel, it is one test. MedlinePlus lists liver panel, liver function panel, liver profile and LFT as names for the same thing. Pakistani labs use all of them, sometimes on the same report.
The plural matters more than you would think. Many people search for "LFTs" rather than "LFT" because that is what the doctor wrote, and they are looking for exactly this test.
What the Test Measures
Your report lists several markers, each pointing at a different part of liver health.
| Marker | What it is | Why it matters |
|---|---|---|
| Bilirubin | Waste from broken down red blood cells | Builds up when the liver cannot clear it, which causes jaundice |
| ALT (SGPT) | An enzyme found mainly in the liver | Rises when liver cells are injured, the most liver specific marker here |
| AST (SGOT) | An enzyme in liver and muscle | Rises with liver damage, but muscle injury lifts it too |
| ALP | An enzyme from liver and bone | Points at a blocked bile duct, or sometimes at bone disease |
| GGT | A bile duct enzyme | Supports an ALP result and is sensitive to alcohol |
| Albumin | The main protein your liver makes | Falls when the liver has been struggling for a long time |
| Total protein | Albumin plus globulins | Shows the overall protein picture |
| A/G ratio | Albumin against globulins | A calculated value your doctor reads alongside the rest |
Normal Range
Read the range printed on your own report first. Mayo Clinic publishes these standard ranges and notes they are typical for adult men, vary from laboratory to laboratory, and can differ slightly for women and children.
| Marker | Standard range |
|---|---|
| ALT | 7 to 55 U/L |
| AST | 8 to 48 U/L |
| ALP | 40 to 129 U/L |
| GGT | 8 to 61 U/L |
| Albumin | 3.5 to 5.0 g/dL |
| Total protein | 6.3 to 7.9 g/dL |
| Bilirubin | 0.1 to 1.2 mg/dL |
What High and Low Values Mean
No single value settles anything. Doctors read the pattern across all of them, together with your history, your medicines and how much alcohol you drink.
| Finding | What it may suggest |
|---|---|
| High ALT and AST | Liver cells under injury, seen in hepatitis, fatty liver and some medicine reactions |
| High ALP with high GGT | A bile duct problem rather than a liver cell problem |
| High ALP with normal GGT | More likely bone related than liver related |
| High bilirubin | The liver or bile ducts are not clearing waste, which shows as jaundice |
| Low albumin | Long standing liver disease, though kidney and gut conditions also lower it |
A mildly raised result on its own is common and often temporary. If you have been unwell recently, your doctor may simply repeat the test later.
Who Should Get Tested and When
MedlinePlus lists the symptoms that should prompt a liver panel.
- Yellowing of the skin or eyes
- Dark urine or pale stool
- Nausea, vomiting or loss of appetite
- Tiredness and weakness that will not lift
- Pain or swelling in the abdomen
- Swelling in the ankles and legs
- Frequent itching
You may also need one without any symptoms if you have a family history of liver disease, hepatitis or exposure to it, diabetes, obesity, heavy alcohol use, or if you take medicines that can affect the liver.
How to Prepare
- Fast for 10 to 12 hours before the sample unless your doctor says otherwise
- Water is allowed during the fast
- Tell your doctor about every medicine and supplement you take, since many shift these numbers
- Avoid alcohol for at least 24 hours before, because it lifts GGT sharply
- Do not stop any prescribed medicine unless your doctor tells you to
What Happens During the Test
- A trained phlebotomist draws a small blood sample from a vein in your arm
- The draw takes under a minute
- You may feel a brief sting and see a small bruise afterwards
- You can eat as soon as the sample is taken
- Your Marham Lab report is ready the same day and reaches you online
Conditions It Can Help Detect
An LFT is a screening and monitoring test, not a diagnosis on its own. It helps look for hepatitis, fatty liver disease, cirrhosis, bile duct blockage and damage from medicines, and it is used to follow a known liver condition over time.
Hepatitis is the reason this test matters so much here. The World Health Organization reports that around 47 million people live with chronic hepatitis C, that the Eastern Mediterranean Region, which includes Pakistan, carries the highest burden at 12 million, and that chronic infection often stays undiagnosed until serious liver damage appears decades later. A liver panel is one of the first places that damage becomes visible.
If your results point that way, your doctor may add Anti HCV or HBsAg. Both are available at Marham Lab.

Final Thoughts
Liver damage is quiet for a long time, and an LFT is the cheapest way to hear it early. If you have hepatitis in the family, take long term medicines, or have been told your liver is fatty, this test is worth repeating on the schedule your doctor sets. Take the report to a doctor rather than reading it alone, because the pattern across the markers matters far more than any single number.
