Talk to Dermatologist on Nail Plate Damaged

mine right hand thumb's nail plate damaged now, history of trauma 17 years ago, but from that time it's was normal, no pain even looks normal like other nails but now from last 4 year's it's hurt's a lot, 4 times nail removed, phly full nail jura tha mass k sth but 4 years sy half jura h r half ni, is dfa 4th time removal k bd center ma b cutt h, dermatologist ko check krwaya tha wo kh ry thy thk ho jay but ni hora i even noticed no change except it's hurts more than before, mjy triton inje lg ry nailfold ma , 6 dfa lg chuky after every 6 weeks, what's will be the solution, i wanna to get rid of, now tired it's hurts a lot it's right hand can't write properly and even can't hold anything through it, nail can be removed permanently???
