Dear your father needs to be assessed by physician to correctly find out the cause plus start of treatment of hyponatremia if it becomes cause is identified just keep his sodium levels well in acceptable levels avoiding any further will admission in hospital and some investigations till then keep sodium level as close as normal get the investigation of origin disease and start treatment for that as well .

Kindly get serum electrolytes done . Improve salt intake by ORS so it will
not increase sodium level instantly . Moreover does he sweat more or
urinate much or vomitings ? How is the appetite and sleep . Any bed sores
?
Or u may get his investigations previously done also and visit my clinic or
any neurologist .
