its better to take insulin twixe but you can change meal habits , consult nutritionists for further

Not usually..70/30 can only be given twice daily as it is an intermediate acting cintrol is not gained dose can be adjusted or another long acting insulin Lantus can be added

if sugar is under control then no need to change the doses
if its not so,he may take a hypoglycemic agent in middle of day
