Asking For Father, Male 63, Islamabad
hi there, my father is an diabetic patient. he had an infection on his foot a year ago. which is still not Cured. we've tried a lot of medication, daily cleaning, sugar control and less walking. but still it's not completely curing. please advise.
kindly, keep his glucose level record and have an examination by the podiatrist.Also, share the image of his foot so that condition can be assessed.

Since diabetic patients are more prone to having infections because of low immunity and more limb problems because of lack of blood supply, from the limited history you have given it could be a case of diabetic foot, diabetic ulcer or gangrene ( wet or dry,)... Kindly share the picture of the wound because the patient might need surgical intervention in addition to medical treatment as well
Diabetic foot management should be managed by a Gen. surgeon or sometimes with opinion of a Plastic Surgeon too. It has too be examined before making any comments. Better book an online consultation with the concerned surgeon.

diabetic foot ulcer or non healing ulcers needs special care.
US doppler of that infected leg , pus and blood Culture test is required for further therapy.
meanwhile wash his infected legs with KMNO4 pinky wash twice a day and apply fusidin cream and do daily bandage.
hi first of all the blood Glucose level should be control.start
tab.amclav 1gm twice daily for 7 Days.
tab.flagyl 400mg thrice daily for 7 days.
tab.chymrol fort twice daily for 7 Days
tab.voltral 50mg for pain
daily aseptic dressing.
IA it will be improve if not improved then consult an orthopedic surgeon which will do x.rays .
without having a look at the wound cant comment on further treatment.. kindly send pic of the foor plz ..

Hello.. First of all, the wound has to be assessed physically. How deep it is, whether infected or not currently.. whether the bone is involved or not.. need to address if the skin tissue is still viable or dead.. I think before any suggestion or treatment, the wound has to be examined thoroughly.
meanwhile continue regular dressing and sugar should be controlled well..

Appears to be dry gangrene. If on pressing the wound there is any secretion, you need to get culture and sensitivity done. Get a cbc and esr meanwhile amd an xray of foot to rule out bone involvment
For treatment there needs to be debridment of dead tissue followed by serial dressings. If there is an indication if concurrent infection only then we need to start oral antibiotic. Keep foot warm and dry. Avoid pressure on the site of ulcer. Optimise blood sugar levels

Check his blood sugar levels daily and keep it's record.Is he on Insulin or tablets for control of diabetes?
Do post a picture of the affected area and get his X-ray done.
Management includes daily dressing,control of diabetes and control of infection.

he is on insulin along with tablets. average sugar levels is now a days from 160 to 270

His sugar levels need to be controlled to below 150 mg/dl.For this , I ask you to tell the dose and timing of insulin and tablets.Also tell which insulin and tablet(name) is he using.Do record blood sugar levels before breakfast and 2 hrs after lunch and dinner after dose adjustment.

clean the wound daily
change dressing n apply fuciderm on the wound
control the sugar level must not exceed from normal
Take good wound care
consult with some general surgeon n take opinion from an orthopedic surgeon as well
continue the medication precribed by diabetologist
share the pic to evaluate the degree n diagnose as well as further treatment..
You need proper consultation involving diabetic foot clinic, endocrinologist, orthopedic and vascular surgeon. To assess the degree and depth of wound.
Get US Doppler of foot.

Its something not to be taken easily and not because it's for a long time but also because of the consequences as it can lead to
-septic shock- as fever , low blood pressure
amputation without septic shock -(secondary to infection or gangrene)
please strict control of sugar
good foot care n hygiene (using foot mirror, comfortable but closed shoes, )
follow up with the doctor physically before it leads to more serious consequences.
