Overall, improved appearances in left ischioanal fossa with no residual drainable collection. However, the fat here is significantly indurated in keeping with ongoing inflammation. An internal opening is noted in lower anal canal, below the dentate line at 1 o'clock. This extends to Y-shaped trans-sphincteric fistula-in-ano communicating with left posteromedial labia with an additional branch blindly terminating within left ischioanal fat.

Appearances are those of a St James' grade IV transsphincteric fistula with secondary tract formation.

No supralevator extension or proctitis. Retroverted uterus.

Colorectal surgical opinion recommended.
