Please get your trans vaginal ultrasound by a good qualified radiologist. If endometrial thickness is confirmed more than 4 cm, you must go hysteroscopy snd D an C and thorough curettage to remove polyp and take good specimen from all walls of endometrial cavity with removal of polyp. The specimen must be subjected to histopathology to rule out endometrial carcinoma
go for good senior radiologist and have TVS for endometrial thickness
AOA,, most probably it's polyp, as it's asymptomatic so for now ek TVS krwae . Kisi bhe procedure ki zarorat nh.

You need biopsy for proper evaluation
need polypectomy and D&C
Endometrial thickness refers to the measurement of the endometrium, which is the inner lining of the uterus. In postmenopausal women, an endometrial thickness of more than 4-5 mm is often considered abnormal and may warrant further investigation, especially if accompanied by any symptoms such as abnormal bleeding.
Symptoms and Signs
In many cases, especially in postmenopausal women, patients may not present with any symptoms. However, some symptoms that could be associated with abnormal endometrial thickness or polyps may include:
Abnormal vaginal bleeding (including spotting)
Unusual discharge
Pelvic pain or discomfort
Urinary symptoms (e.g., burning sensation)
Investigations
The following investigations may be used to evaluate endometrial thickness:
Ultrasound: Transvaginal ultrasound is a common first-line imaging technique to assess endometrial thickness and look for polyps or other abnormalities.
Endometrial Biopsy: This is often recommended if there is increased thickness, especially in the absence of menstruation, to rule out endometrial hyperplasia or malignancy.
D&C (Dilation and Curettage): This procedure can be done for both diagnostic and therapeutic purposes. It allows for the collection of endometrial tissue and can also help remove polyps or abnormal tissue.
Management
Given your history of menopause and the recent finding of an 8.1 mm endometrial thickness with a polyp, the following management options may be considered:
Monitoring: If there are no symptoms and the ultrasound findings are stable, some doctors may choose to monitor the endometrial thickness with follow-up ultrasounds.
Endometrial Biopsy: As advised by your gynecologist, an endometrial biopsy is generally recommended to ensure there are no precancerous changes or malignancy, especially given the polyp.
Surgical Intervention: If the biopsy results indicate hyperplasia or cancer, or if the polyp is symptomatic, surgical intervention such as hysteroscopy to remove the polyp may be necessary.
Prevention
Postmenopausal women should be vigilant for any unusual changes in their menstrual cycle, even if they are not currently menstruating. Regular health check-ups with a gynecologist can help in early detection and management of potential issues related to endometrial health.
Conclusion
In your specific case, the recommendation for an endometrial biopsy and diagnostic D&C is based on an evidence-based approach to rule out any significant pathology, despite the absence of symptoms. It is important to discuss your concerns and any specific questions with your healthcare provider for personalized advice.
incteased endometrial thickness with a polyp needs either hysteroscopy or diagnostic D& C .it should not be neglected.
