Talk to Gynecologist on Peruods Issue

My periods dont start from regualr flow from 8 to 9 years .I have done my ultar sound it was the start of my periods in first 7 days there is just spotting along with much pain and heavy head these days heavy bleeding start for about 6 to 7 days then again spotting .In this way thi process lasts for about 18 days suggest me what can I do now main issue is severe pain and headach during spotting
