Asking for Someone Else, Sister, Female, 19 years old, Rawalpindi

Assalam-o-Alaikum, Doctor. I am a 19-year-old unmarried female from Pakistan. I was diagnosed with PCOS at the age of 15. I had my first period at the age of 12, but my menstrual cycles have always been very irregular and usually became regular only while taking prescribed medication.

Last year, after stopping all medications, my periods surprisingly became regular on their own for around 10 consecutive months. After those 10 months, I skipped one month, and the following month I had bleeding for around 4–5 days, but it was mostly very dark brown blood/discharge rather than normal red menstrual bleeding. Then I skipped another month, and the following cycle I had normal red bleeding for only 2–3 days, followed by brown bleeding again. After another skipped cycle, I had a proper normal period in April (around 16 April) with normal bleeding and significant cramps.

On 25 April, I had one episode of unprotected intercourse with withdrawal (ejaculation occurred outside). I have had no intercourse since then. I performed multiple home urine pregnancy tests: two during the last week of May, one during the first week of June, and two more before the middle of June. All of them were negative.

Over the past few weeks, I experienced intermittent nipple tenderness, mild lower abdominal/right-sided discomfort, temporary mood changes, and occasional lower back discomfort. Most of these symptoms have now improved, but I still have not had another period since April.

My gynecologist had previously advised me that if I did not get my periods for 2–3 months because of PCOS, I could take Primolut N to induce withdrawal bleeding. If I repeat a home pregnancy test now and it is also negative, would it be appropriate to follow my gynecologist’s previous instructions and start Primolut N? Thank you.

Serum beta hCG is not possible for me at the moment. I have already had multiple negative home pregnancy tests. If I repeat one more home test and it is negative, can I follow my gynecologist’s previous Primolut N plan?
