Asking for Sister, Female, 35 years old, Islamabad
My sister is 35 years old. She has primary amenorrhea (Low FSH and LH;, Normal prolactin and thyroid hormones; normal ovaries, but uterus small in size) and she is on birth control pills. She is taking these hormonal pills from age 21 and took for about 3-4 years. Then stopped taking these pills. Then in 2016 November, she had pain in bones and undergone DEXA Scan and vit D test as recommended by orthopedic doctor. Results showed low vit. level and osteoporosis in spine and osteopenia in forearm. Orthopedic Doctor gave her Strontium renelate for 3 months+vit D, Calcium supplements. And said no need for further osteoporosis treatment and recommended to use supplements+hormonal pills+exercise and walk. Started again hormonal pills from 2016 November as recommended by gynecologist for bones. Currently, last month 2019, she undergone DEXA scan again, and results showed Osteopenia in spine, hip and forearm. Endocrinologist doctor recommended her Bisphosphonates treatment for osteopenia. Cu

-Dear she need Detailed Evaluation Endocrinologically for her all hormonal issues.
-long term contraceptives could not be given as having side effects like even Cancer.
-have balanced diet, weight and Bmi.
-Complete treatment for any health issue u have.
-have recreational activities and quit recreational drugs.
-have Exercise, walk, yoga.
-have calcium with Vit D regularly.
-have physiotherapy, Exercise etc.

Dear Doc, I want to ask about her treatment for osteopenia? should she start treatment using bisphosphonates now at this age or wait? and for primary ammenorrhea, should she continue using birth control pills and for how long?
One more question is, in 2016 when she undergone Prolactin test, it was 107.77 mIU/L without using birth control pills for years. Is this normal or increased prolactin level and is it associated with prolactinoma?
After using birth control pills, now last month prolactin is 15.56 ng/mL.
Kindly guide.

Here in Pakistan, doctors are confused, and they don't know what to do with her in this condition i.e osteopenia/ osteoporosis at this age. Both Gynecologist and Endocrinologist just recommended her to use birth control pills. Can she reverse osteopenia using natural food products, etc.

currently she visited Endocrinologist, she only recommended her bisphophonatess for osteopenia, and nothing else.
