Talk to Cancer Specialist / Oncologist on Breast Surgeon (oncologist) Advice Required

Asking For Mother, Female 72, Lahore

My mother has been diagnosed with breast Cancer, we have consulted oncoplastic Surgeon in Lahore and planned Lumpectomy on 21st July. We are confused about whether we opt for a mastectomy as I have read that it is possible that a lumpectomy surgery can be required again. Moreover, radiation will also be essential after a lumpectomy.

please look into attached reports of ultrasound, mammography and biopsy and guide whether lumpectomy is okay and repeated surgeries and radiation can be avoided after lumpectomy?

which one has less side effects and preferred treatment?

both are viable options.
both have their pros and cons.

normally younger perfer lumpectomy due to cosmetic issue. Older patients some time not bother it.
it patients discretion and choice. If you want to avoid radiotherapy in case indicated after lumpectomy, then go for mastectomy. choice would be yours.

Note: If you are confused, there is no harm for second opinion from Surgeons, they give you better inside of procedure

It is an invasive disease and there is no mention of axilla at all. I would personally recommend mastectomy arm at this age, and yes it preferably include axillary staging. This because, it might help skip adjuvant XRT. But in case you're so confused, try communicating with Oncologist in your access and try Breast surgeon vs Surgical Oncologists to add to your decision, especially if your current team did not mention the axilla.

Jazzak Allah Dr sb, yes we have consulted oncologist Surgeon and he has planned lumpectomy along with intraoperative identification of sentinel lymph node on 21st July.
he also told that there is no need of radiation in the light of latest clinical trails CALGB nine three four three and prime 2 etc

we have also consulted clinical oncologist, he just added that radiation might be needed after surgery but it depends on the after surgery biopsy report.

at the age of 72 lumpectomy with radiation has low consequences or mescotomy has low consequences?

Dear, in general lumpectomy carries more risk of cancer coming back. So, it is consolidated with XRT.
I would like clinical oncologist to supervise this in more detail if I were in your footsteps. Prime 2 says both groups had equal OS at the expense of more recurrences in people who skipped XRT. Moreover, go to clinical oncologist to ask what in your case is a contraindication to XRT. Is there any? Please keep in mind you don't know the outcome. But being too optimistic that XRT won't be needed at all, is a relative overstatement. Please be humble and get reviewed from oncologist that your diagnostic workup is complete and meets the optimal criteria. Allah ap k liyay aasani farmaye. Ameen.

Lumpectomy versus mastectomy result of both are equal. but with lumpectomy as u r already aware that post op radiation is mandatory. Now a days their is deescalation of previous surgical method every field is growing and with surgical techniques many women are cured with lumpectomies

Thanks Ma'am for your such a kind and detailed reply, we have consulted breast oncoplastic Surgeon in Lahore, he was saying that latest studies/trails like CALGB 9343 & PRIME II shows that radiation can be omitted in females over 70 years age.
please guide are we on the right path?
