It's always better to have an opinion after physical consultation from a physician related to field. I would advise you seek Dr Tariq in Khyber Teaching Hospital.
I have not physically assessed the patient but I would recommend to have a pleural tap and cell block assessment if possible. Getting pt physically assessed would clear out the doubts. If cell block isn't possible then Biopsy is necessary as advised by Dr Fareeha.

Assalam alaikum according to your sister CT report and marked raised CA-125 it’s probably Epithelial Ovarian Cancer so contact Good oncology surgeon for surgical staging and work up of ovarian cancer. Thanks
Unfortunately CT shows advanced ovarian cancer. I will recommend taking fluid out from abdomen and if shows abnormal cancer cells, then to offer her chemotherapy for ovarian cancer by cancer specialist near your home town. As there is fluid in her lungs and lymph nodes in chest, do I will not recommend surgery but can consider if there is a very good response to chemotherapy treatment
