Salam kindly get his pet ct scan done along with basic blood work cbc uce lfts ldh uric acid calcium phosphate Hbsag antihcv .look like Hodgkin lymphoma.
Salam
Kindly review biopsy from another lab and then proceed accordingly
The biopsy is inconclusive due to scanty material and crushing artifacts; however, immunohistochemistry findings (CD30+, partial CD15+) along with clinical presentation are strongly suggestive of a lymphoproliferative disorder, most likely Hodgkin lymphoma.
➡️ Recommended Next Steps:
1. If feasible, proceed with a repeat biopsy, preferably excisional or core biopsy from a larger, intact lymph node, to obtain adequate tissue for definitive diagnosis.
2. If re-biopsy is not possible, then proceed with:
PET-CT scan for metabolic activity and staging.
Echocardiography to assess cardiac function before chemotherapy.
Baseline laboratory workup including:
CBC with differential
ESR, LDH
Liver and renal function tests
Viral markers (HBV, HCV, HIV)
Electrolytes
3. If the patient is hemodynamically stable and has acceptable performance status, initiate chemotherapy per Hodgkin lymphoma protocol (e.g., ABVD regimen) after multidisciplinary discussion.
