Asking for Father, Male, 70 years old, Karachi
My father was diagnosed with a tumour in bladder in September 2019. After that he had gone through turbt and biopsy report reveals zero stage bladder cancer. For which he got 6 BCG shots. After a gap of 1.5 months he had gone through cystoscopy again and biopsy reveals that tumour has enlarged and change into a high grade carcinoma. Now he is taking treatment from a Dr . At first Dr said there will be three weeks chemo session followed by 33 days radiation. But now he is a saying k there will chemo for total 9 weeks. We have taken second opinion and mostlr drs said that chemo of three weeks is enough. So Kia aaap bata saktay Hain k generally is stage Mai chemos kitny honey chahye as it has to be followed for radiation (and then surgery if needed.)
first of all send the biopsy report of high grade tumor then I will tell you the final plan of treatment

unfortunately you have not attached any pathology or Radiology or cystoscopy report which can exactly determine the state of affairs.However
as it is a recurrent disease so we have many options. Proper assessment of patient's condition is another issue at this age. Giving first Radio& then chemo for longer period is not a bad option so far if pt is responding on cysto/or CT followed by surgery
I think you are talking that the disease has now progressed and he now has locally advanced bladder cancer.
Generally it is treated with chemo-radiation and radiation is obviously 30-33 fractions but chemotherapy regimes vary. It may be 3 weekly chemo for 3 cycles or weekly for 7- 9 cycles.
We prefer to give weekly chemo in elderly patients as it decreases toxicity but still both regimes are acceptable.
Is it muscle invasive or localized. What is the stage of disease now?

So sorry Dr's I tried so many times to attach tha biopsy and cystoscopy reports but I am unable to attach it . The site is not accepting the files . Thank you very much for your responses
Yes the tumour is invasive now and it is increasing in size also

In first biopsy report done in sept 2019 it was mentioned as " high grade papillary urothelial carcinoma invading lamina propria" and now it is termed as "Infiltrative malignant and neoplastic lesions composed of nests".
Need to see all reports.
Chemotherapy doses and options could vary depending on disease involvement and condition of the patient.
