Known case of HTN for last 5 years and on anti hypertensive[Amlodipine].
Prediabetic with HBA1c of 5.9.
No any Cardiac illness on Echo.
P/C: Back pain for 1.5 years Pain in Shoulder for 1.5 years
HOPC:ATSOMM she was in usual state of health 1.5 years ago then she develops pain which is sudden in onset burning in character. Pain is aggravated by work like doing dishes and walking.
Relieved by rest and medicatins.
No any hx of injury or accident.
No tingling and numbness.
She is Obese with a 94 kg weight.
She works a physically demanding job link Washing clothes,doing dishes and lifting heavy weights things in home for years.
INVESTIGATION Her MRI shows Multilevel disc disease with degenerative changes noted especially at L3-L4, L4-L5 & L5-S1 levels as described above Her EMG/NCS shows L5-S1 Rediculopathy All other investions are normal ex

No Physiotherapy is performed
Only a device named TENS[Trans electric nerve stimulator] is being used on affected sites.
Now its been A month she is taking these Medications
And a little amount of Pain is relieved
But she develops Edema in both legs.
She just stop taking medications since 2 days.
No any Neurological Examination is done in this Case

very detailed history is given.
if there is no timgling numbness or focal neurological signs, then surgery is not recommended.
conservative management can be successful but treatment will be prolonged. including traditional pain killers and those for neuropathic pain.
activity and life style modification might help. like reducing weight and avoiding squatting and other strenuos activities.
prognosis can be satisfactory if you take necessary precautions and pain killers on a long term basis. hope it helps.
