Dear Doctors,
I hope you are all doing well. I am a 25-year-old patient with a history of Juvenile Idiopathic Arthritis. Over the past ten years, I have developed bilateral hip damage with a hip-flexion contracture of approximately 40–45°. My current medications include Methotrexate (oral) with Inercipt injections and Folic Acid; previously, I was on Sulfasalazine EC.
My height is 5 ft and my weight is about 54–55 kg. I currently use a wheelchair and have minimal ability to stand or walk.
I am now planning for total hip replacement surgery. Different physicians have recommended different bearing surfaces—some suggest ceramic-on-ceramic while others recommend a metal (alloy) option. I am uncertain which would be more suitable in my case.
Could you please share your professional opinion on the advantages, potential risks, and long-term outcomes of these options for me? Your guidance would mean a great deal to me.

as you're young and advanced arthritis, you should go for uncemented total hip arthroplasty , ceramic on ceramic is brittle so the chances of breakage is more but it's wear is negligible so if handled properly it's the best option for you. alloy implants are better in strength but it has the problem of early wear comparatively. ceramic on ceramic is a better option for you.

can you please share the series of various xrays and let me what are the main indicators of THR at this stage, you are young and surgery at this stage mean. you may needs few surgreis until the age of 50.
