Asking For Father, Male 61, lahore

Hi! My father had an accident on 15th July, 2025 and got surgery done (16th July). His Tibia got 4 fractures. He is diabetic and the doctor put an external fixature.Now after two months, when we did X-ray, we feel there is no improvement. I consulted 2 more doctors and they said metal rod should have been done. At the time of accident, only he had a severe wound on foot while on leg there were only bruises no wounds. Our own doctor is saying to wait for four more month else we will do bone grafting. Please suggest, what should be done. You can see and compare the Xrays.

nailing with bone graft would be more stable after ruling out infection.
exfix would not be much stable. you can wait for some time so that some union occurs.

nailing is better option if there is no infection otherwise May need Elizrove fixation.....present fixation good for only temporary fixation may not help in long run

Thank you for detailed and appropriate information.
No point in discussing what was done by the attending surgeon, what he did was right.
Chances of delayed union and non union are more with external fixator as a definitive treatment plus your father is Diabetic as well.
Now your father needs definitive fixation .Options are
Nail or plate , whichever implant Surgeon use, he should remove the external fixator and do definitive fixation once the pin sites are clean and dry ( may take 4 weeks)
Distal third of Fibula fracture can be fixed for better stability ( it's an option)

Removal of EX-Fix, Swab from pin sites for CS,CBC, ESR, CRP before removal, wait for 4 to six weeks, Control diabetes, repeat inflammatory markers ,if normal, go for ILN, or Distal tibial locking plate.
