yes I've skipped as doctor has adviced me for only one month and again asked me for check up. But due to death at my home I couldn't get another appointment which was supposed to be on 3rd of August

One thing which you are are doing wrong from the begining, instead it's because of your psychiatrist, it's that you are only taking medicine to treat your depression or anxiety (as for now, I do not know your symptoms). Mental related problems are treated with medication PLUS "PSYCHOTHERAPY". After taking psychotherapy you will not require medication in long run.

actually I started with a Psychologist's therapy 6 months ago. had 3 sessions and had a very painful experience. Reminding to those past life events I became very sick that was admitted to hospital due to stress.
