Talk to Psychiatry on Existential Ocd Medication

person-avatar

Asking for Self, Male, 36 years old, Islamabad

Using paroxetine cr 37.5 mg , opanzapine 5 mg and rivotril 0.5 mg. Most days are better but sometimes have strong relapse where I don't want to get up from bed whole day. Can this medication be further adjustment, if yes what can be options that can be discussed with the psychiatrist.

How long have you been taking these medications at the current doses? It’s good that you’re feeling better on most days, but since you’re still having significant relapses, your treatment may need to be reviewed. Your psychiatrist may consider adjusting the medication or exploring other treatment options. Please discuss this with your treating psychiatrist and don’t make any changes on your own.

Existential OCD often requires higher doses of SSRIs than depression, and while paroxetine CR 37.5 mg is a therapeutic dose, there may still be room for optimization depending on your response and tolerability. It is important to discuss with your psychiatrist whether a dose adjustment, a change in medication, or augmentation (e.g., with another evidence-based agent) is appropriate. Equally important is Exposure and Response Prevention (ERP)-based CBT,which is considered the most effective psychological treatment for OCD. Since you are experiencing recurrent relapses with significant impairment, I would recommend a follow up with your psychiatrist for reassessment rather than making any medication changes on your own.

Today I added risperidone 1 mg instead of opanzapine and found great relief even with a single dose
