Walaikum Assalam. It is good that you are noticing which medicines help, but please avoid changing doses on your own,as it can disturb your progress or cause side effects. Discuss your current response and restlessness with your psychiatrist he/she can safely adjust Kempro or Dosik if needed. Staying consistent under one doctor’s guidance will help you achieve stable, lasting improvement.

It seems like you're looking for information on the medications you mentioned and potentially others that might be suitable for treating depression and related conditions, especially after having had a good response to certain combinations but facing medication changes. I'll provide a brief overview of the medications you've mentioned, as well as some general information on psychiatric drugs typically used for depression and their profiles.

Medications Discussed

Zylex SR (Escitalopram)

Indication: Major Depressive Disorder, Generalized Anxiety Disorder.

Mechanism: Selective serotonin reuptake inhibitor (SSRI) that increases serotonin levels in the brain.

Contraindications: Hypersensitivity to escitalopram or citalopram, concomitant use of monoamine oxidase inhibitors (MAOIs).

Side Effects: Nausea, insomnia, fatigue, sexual dysfunction, dry mouth.

Adverse Reactions: Increased risk of suicidal thoughts in young adults, serotonin syndrome.

Dowvigil (Modafinil)

Indication: Primarily used for narcolepsy, sleep apnea, and shift work sleep disorder.

Mechanism: Promotes wakefulness by affecting neurotransmitters in the brain.

Contraindications: Severe hypersensitivity to modafinil or armodafinil, history of valvular heart disease.

Side Effects: Headache, nausea, anxiety, insomnia.

Adverse Reactions: Serious skin rashes, psychiatric symptoms like anxiety.

Attentra (Atomoxetine)

Indication: Attention Deficit Hyperactivity Disorder (ADHD).

Mechanism: Selective norepinephrine reuptake inhibitor (NRI).

Contraindications: Narrow-angle glaucoma, severe cardiovascular disorders, MAOIs.

Side Effects: Decreased appetite, dry mouth, fatigue, mood swings.

Adverse Reactions: Increased risk of suicidal thoughts, particularly in children and adolescents.

General Considerations for Depression Management

SSRIs & SNRIs: Other SSRIs (like fluoxetine, sertraline) or SNRIs (like venlafaxine, duloxetine) may also be considered if previous medications were ineffective.

Atypical Antidepressants: Bupropion or mirtazapine could be alternatives, particularly for patients with fatigue or insomnia.

Psychotherapy: Cognitive-behavioral therapy (CBT) or other therapeutic modalities can be effective adjuncts or alternatives to pharmacotherapy.

Lifestyle Modifications: Regular exercise, diet, and sleep hygiene can also improve depressive symptoms.

Medication Management: It’s essential to closely monitor for side effects, drug interactions, and the overall efficacy of the treatment plan.

Self adjusting dosage can be risky so its better to follow medication prescribed by your psychiatrist. alongside medication. Regular therapy, relaxation training and a consistent daily routine can improve your depression and anxiety in long-term.
