Asking for Someone Else, Male, 33 years old, Rahim Yar Khan
I’m a 33-year-old unmarried male. I’ve been struggling with several mental and physical symptoms for around 4–5 years, which became more noticeable after a breakup in 2021.
I constantly overthink, even about small things, and my mind feels occupied almost all the time. I experience brain fog, poor concentration, lack of mental clarity/focus, and a feeling of heaviness/pressure in my head and face, sometimes like a “magnetic field.” I also experience anxiety, social anxiety and fear of being judged. Even before posting something normal online, I may worry that someone I know will see it, laugh at me or judge me. I have intrusive thoughts/waswasa, low self-confidence, feelings of being inferior, and I can be easily affected by other people's opinions. I often take small things personally and keep thinking about them repeatedly. I also experience irritability, lack of motivation and difficulty enjoying life. Sometimes I feel completely normal, but the symptoms return.
Along with this, I have developed significant GI problems, especially severe/chronic constipation, and I have also noticed premature greying of my hair. Overall, these problems are affecting my quality of life, concentration, confidence and ability to enjoy normal life. My main questions are: Could this be anxiety, social anxiety, depression, OCD/intrusive thoughts, or a combination? Could my gut problems be contributing to my anxiety/brain fog, or could chronic anxiety and stress be causing or worsening my gut problems? Is the gut-brain connection relevant here? Should I consult a psychiatrist, clinical psychologist, gastroenterologist or physician first, or should I see more than one specialist? Would CBT/psychotherapy be helpful, and when is medication considered? Should I have basic tests to rule out physical causes such as thyroid problems, anemia or vitamin deficiencies? just want professional guidance about the possible causes and the right specialist/treatment
Your symptoms sound quite distressing, and when overthinking, anxiety, intrusive thoughts, low confidence and physical symptoms continue for years, a proper assessment is important. These symptoms can overlap with anxiety, social anxiety, depression or OCD, but a diagnosis cannot be made without a detailed evaluation.
I would recommend starting with a psychiatrist or clinical psychologist for a comprehensive assessment, while a physician/gastroenterologist can evaluate your chronic constipation and rule out physical causes. Basic medical tests may also be considered based on your history.
CBT and other evidence-based therapies can be very helpful for anxiety, intrusive thoughts, overthinking and social anxiety. If you would like, I can assess your symptoms and help you develop a structured therapy plan.
