if she is bed bound or stays in bed for more than 50 percent of waking hours i think we should not add to her misery .but still can add palliative options like oral pain killers, nerve block or palliative stenting for jaundice as required so as to keep her symptom burden at minimum as much as we can .
if her condition improves with supportive care than first thing to do will be biopsy from the most accessible site .
