I’m 19 and have been experiencing several symptoms for a while. Since this morning, I’ve had a burning feeling in my chest/heart and upper stomach for about 4 hours. I’ve been nauseous and vomited once.

I also frequently get sudden episodes of very strong heart pounding/thudding. It feels like my whole chest vibrates with each beat, and sometimes my breath feels stuck. This happens randomly and almost daily. Previously, I’ve also experienced brief numbness, my vision going black, and feeling like I might faint.

Other symptoms include persistent fatigue and sleepiness, poor appetite, nausea after eating/drinking, difficulty gaining weight, frequent headaches, unusual bruising and small red spots on my skin, and swollen/bleeding gums. I also have significant period cramps and general body aches.

I’m worried these symptoms could be connected. Could you please advise whether I need an urgent checkup and what tests I should have, such as an ECG or blood tests?

Initial assessment: * 12-lead ECG — essential because of palpitations/near-syncope. * HR, BP, SpO₂, temperature and orthostatic BP/HR * CBC with platelet count — particularly important because of petechiae, bruising and bleeding gums * Ferritin/iron studies if anemia suspected * Electrolytes, renal function, glucose, Mg ± Ca * TSH/free T4 * PT/INR, aPTT ± further coagulation/hematology testing depending on results * Pregnancy test if applicable * Troponin if the clinical assessment indicates possible myocardial injury * Echocardiogram if ECG/examination/history suggests structural disease * Holter/event monitor if the ECG is normal but episodes continue; ideally capturing an episode is very useful
