Talk to Urologist on Urgency When When I Feel The Need To Urinate

person-avatar

Asking For Self, Male 32, Lahore

I have been experiencing this problem for a long time. I’ve tried various remedies for it, and while they do help, the relief is only temporary. A few months ago, I visited a doctor who prescribed me Mirabet and diagnosed me with hyper active bladder. I used it for about a week, and it was effective at first, but now the condition has returned to the same as before.

Whenever I feel the urge to urinate, I find it difficult to control the urine, i feel like i need to pass it right away(as if i may pass it right now), the urgency to urinate is felt even when it's just normal flow while urinating. During cold weather, I have to urinate frequently and this feeling is intense. Even the slightest cool weather in summer, my legs get cold and that cold feeling adds to the frequent urination.

About two hours after drinking coffee or tea, the urge to urinate becomes much stronger.

I need to fix my condition permanently. Please prescribe me medicine.

From your detailed description, you are experiencing symptoms consistent with overactive bladder (OAB), characterized by urinary urgency, frequency, and a sensation of incomplete bladder control. Your previous prescription, Mirabet (mirabegron), which is a beta-3 adrenergic agonist, temporarily helped by relaxing bladder muscles and increasing bladder capacity. However, OAB is often a chronic condition that may require a multi-faceted approach beyond just medication for sustained relief.

Given your symptoms intensify with cold exposure and stimulants like coffee and tea, lifestyle modifications are crucial. Avoiding bladder irritants such as caffeine and minimizing cold exposure to lower extremities can help reduce urgency. Bladder training exercises, including timed voiding and pelvic floor muscle strengthening (Kegel exercises), are strongly recommended to improve bladder control over time.

From a pharmacological standpoint, if mirabegron alone is insufficient, combined therapy with antimuscarinic agents such as oxybutynin or tolterodine might be beneficial, but these come with potential side effects like dry mouth and constipation, so careful monitoring is needed. Since you report a return of symptoms after a short course, a longer treatment duration under medical supervision is generally advised.

Furthermore, considering your persistent symptoms, it is important to exclude other potential causes such as urinary tract infections, bladder stones, or prostate issues. A referral to a urologist for further evaluation including urine tests, bladder scans, or cystoscopy might be necessary.

In summary:
1. Continue or restart mirabegron under your doctor\'s guidance, often for several weeks or months.
2. Incorporate lifestyle changes: reduce caffeine intake, avoid cold exposure especially to legs, practice bladder training.
3. Discuss with your healthcare provider the possibility of adding or switching to antimuscarinic medications if symptoms persist.
4. Seek specialist evaluation to rule out other underlying conditions.

I advise you to schedule a follow-up appointment with your healthcare provider or a urologist for a comprehensive assessment and personalized treatment plan rather than self-medicating. Proper diagnosis and management based on your clinical examination and investigations are essential for a permanent solution.
