Talk to General Physician on Fungal Infection

Hello. My grandmother has a fungal infection on her toe nail. She is really worried as different doctors are saying different things and giving her ointments and stuff but its not getting any better. Please suggest what this is and whats the treatment? Also her toe is also swelling. I have attached the picture
