Asking for Self, Male, 30 years old, Faisalabad

I have occasional squeezing chest pain above food pipe, Once I got done ECG at emergency and that was normal, doctor diagnosed it as GERD, and prescribed Esomeprazole 40mg twice a day, Levosupiride 25mg Three times a day for 2 week and the pain was gone. but it not gone completely, it occurs occasionally. I eat two times in a day. I'm also hypertensive which is well-controlled. my blood pressure is 130/80 or less.

I was wondering, is not any anti-hypertensive the cause/worsening of GERD?

I take following drugs for high blood pressure, Tab. Norvasc 5mg ---- One in the Morning Tab. Zestril 20mg ---- One at Night Tab. Inderal 10mg ---- Two Twice a Day Tab. Ascard 75mg ----- Two days per week

GERD Needs life long lifestyle & food modification and if you take too much coffee or tea or oily and spicy food, if your overweight or you smoke all these will cause symptoms to recur, so do lifestyle & dietary changes, Take LOPRIN ENTERIC COATED instead of Ascard as it can cause gerd ,rest of drugs are fine

Ok, Ascard is replaced with Loprin.
I am now down to One cup of tea, I totally avoid fatty, deep fried, baked, sugary and processed/fast foods. I occasionally have one/two Samosa after two/three month.
I never smoked.
I'm having two meals a day.
One symptom forgot to mention, throat irritation, lump in throat feeling, frequent need to clear throat. It happens two/three times every year.
Is it GERD related?

can anyone give justification for use of aspirin in 30 years old hypertensive for primary prevention?
dear,
there are no recommendations to use aspirin unless you have suffered a heart attack or stroke and aspirin is a strong gastric irritant so first of all stop aspirin. then its worth having H Pyloris stool antigen test .continue using omeprazole but in lower dose 20mg bd along with all mentioned lifestyle advise.it seems like you are more sensitive than a lot of doctors out and I am so happy that you have given this thought. drugs side effects are the very common cause of problems and in Pakistan polypharmacy, I mean loading with drugs is an issue. There are a lot of issues with your treatment plan
1/ amlodipine cause acid reflux or make it worse
2/ Inderal has no place in BP control in present-day lot better beta blockers are available
3/dose optimization is needed before considering add-on therapy you are on small doses of too many

I never had heart attack, stroke or TIA etc... thanks to Allah. I've essential Hypertention along dyslipidemia.
For six months Aspirin was giver daily, then it was reduced to two days per week by Cardiologist. Ascard/Loprin is enteric coated aspirin.
H. Pylori was once done and it was negative.
Inderal is for Heart-rate only. Earlier I was given Metoprolol Tartrate 25mg BD.
I'm going to discuss your mentioned all three points with my cardiologist.
When I was six/seven, my speech was normal, I was given some medicines for high-fever, vomiting etc... and drugs reacted and caused me stuttering. So now I give more importance to drug interaction. And never a doctor i visited, whether he/she was Dermatologist, Internist etc... asked me whether I'm currently taking any medicines for any condition. I always have to told them and asked about the interaction.
Once dermatologist prescribed Doxycycline 100mg BD, and never mentioned how to take it, I took with water and lay down, later I got severe GERD - squeezing chest pain. At next appointment I tod the doctor he, didn't knew any such side-effect of Doxy, when I searched the net, It was mentioned to take it with food with full glass of water and don't lie down at least thirty minutes after taking it.
This is the situation in the country.

Just reduce asprine like ascard to once a day,stop spicy,acidic,roasted
food.Meal should contain vegitable and salad.Reduce Tea and Stop cold
drinks. Just use china 200 Homeopathic germon shwabe.5 drops in half glass
of water before half an hour before brakefast.reply after 3 days to inform
about your health.

i absolutely agree look into possible precipitant if this doesn't sort it out take omeprazole /lansoprazole etc you can add prokinetics like metoclopramide but be careful it can some time cause same reaction what you had a child. Research has shown some benefits of baclofen even in severe intractable reflux compromising the quality of life people are tried on long term low dse erythromycin as well

Can u please share all your blood test reports like fasting lipid profile, liver profile and kidney profile? Was the cause of high blood pressure found out before putting you on medicine? Have u had your EEG done?

original diagnosis made by FCPS cardiology was Essential Hypertension. He just checked pulse or something on my right side of thigh at joining point, probably femoral and said its Essential Hypertension. Although I got done Renal Doppler and later Renal Scan at PINUM, which were Normal with comments ''Well Functioning both Kidneys''
Last years Urea/Creat were as;
Blood Urea 25 and Serum Creatinine 0.9.
ECG are normal.
LFT was as;
Bilirubin total 0.9, Alk. Phosphate 220, SGPT 32,
Lipid Profile;
Cholesterol 237, TG 241, HDL 38, LDL 151 (Sample was taken when I wasn't taking Atorva 10mg, When I'm on it, Lipid Profile remains under normal range)
Fasting Sugar 96.
EEG was never done.

Yes I've totally eliminated Oninions, Tomatoes, Lemon, Mint etc... from Salad.
Tea is reduced to One cup a day.
I already abstain from Fast-foods, Pizzas, Burgers, etc...
Eating dinner three hour before bed.
Medicines have been changed to Rabeprazole 20mg 1+1 plus Domperidone 10mg 1+1 for Four week.
