My father is a hypertension and diabetes patient. Of recently he has started having trouble with his b.p he complains the his b.p rises if does any activity with rapid speed. Eg brisk walk. I am attaching his reports. Please mention what could be the possible reason

Tab Concur 2.5mg once a day .
