Dr I am here to ask some questions about my son who is 13 yrs.. if you have some time kindly reply back to me I will be very thankful to you My son has pectus carinatum and few days back he got diagnosed with orthostatic proteinuria
kuch report result yeh hai: serum 25 hydroxy vit D is 19.9 ng/ml 1,25 dihydroxy vit D is 111 pg/ml plasma pth is 71.50 pg/ml serum phosphorus is 6.7 mg/dl spot urine creatinine is 48 mg/dl spot urine calcium is 0.8 mg/dl serum calcium is 9.1 mg/dl serum creatinine is 0.6 mg/dl

Please visit Pedetrician. and also get an opinion with endocrinologist. His PTH is raised with normal calcium. It's hard to provide advice just on seeing the reports. Please also describe if he has any symptoms. For detailed evaluation. Please book an online appointment with a Specialist. Thanks

I visited endocrinologist, she said she is seeing this type of case for the first time, maybe his metabolism is high that's why there is imbalance in his levels and increased his D-Max(vitamin D) tablets to 2...But this was not a satisfying reply from her and still I am confused what's the root cause of all this and that's why I am here...

there were no such symptoms...actually he have pigeon chest and his vit D remains low despite of taking medicine...from last 7 months there was high protein level in his Urine...which was then diagnosed by nephrologist that he have Orthostatic proteinuria...and then she referred to endocrinologist to check his vit D levels and there he went through all these tests...

I totally understand your anxiety at this moment. I have rechecked the investigations thoroughly and calculated his serum calcium with albumin. It's again in normal range. Orthostatic proteinuria can happen after prolonged standing or exercise. You can repeat this test after few weeks just to rule it out. Vitamin D aswell is mildly deficient. You can repeat after one month of giving vit. D supplements. Additionally, may I know, is he applying orthosis for Pigeon test? Please tell about any symptoms so that I can advise you properly.

not applying orthosis for pigeon chest as I have no idea where to consult about it...his pediatrician said when he will grow up and will build muscles it will hide automatically...but I am feeling with time it's more visible as my son is very thin...

Please visit orthopedic surgeon and physiotherapists. Orthosis are very helpful to bring it back into normal shape specially at the age of growth spurt. They are like support belts. Please consult with them so that they can give orthosis based on his age and height.
