Talk to General Physician on Red Spots On Palm

I am suffering from many problems. I am unable to understand to which doctor should I consult. for last two years i am having following symptoms. excessive loss of hair, loss of appetite, no weight gain, confusion and disturbing mood. Sometimes it is so difficult to swallow food. 1 month ago varicose veins appeared on upper abdomen. 3 days ago red spots on palms have appeared with irritation and hotness. please guide me properly.
