After heart bypass done 3 month ago we make one followup from surgeon where they check X ray and suggest some medicine now after 2 and half month do we refer to heart surgeon again or move to heart physician for routine check up

yes , it is good to have a follow up with the dr who did the bypass surgery, when you do so, he may refer you to a cardiologist with whom you should have regular follow up examinations and necessary tests like lipid profile , blood glucose, exercise tolerance test etc.
