Talk to General Surgeon on Piles After Delivery

Asking for Self, Female, 27 years old, Karachi

I got severe constipation and piles after delivering my baby girl on 8th March. Started ispaghol and a herbal ointment called Hadensa along with pain killer bilut nothing worked until I took Lilac. After this the problem was cured for a few days and now ai again have the same issue. This time I was not and am not constipated but the pain is worse. It feels like there is a wound on the opening which is aching so bad. I am unable to even sit even though I am having ispaghol and pain killers daily. Have also been using xyloaid antibiotic ointment and hadensa forte but the pain is just the same. Please help. Also please mention a pain killer that will work till the problem is cured.

Asa you have most probably have a fissure that is a crack in the back passage. The best would be to present in OMAR HOSPITAL clinic where I can confirm the diagnosis and treat u effectively.

It can be anal fissher. Treatment is that you apply G.t.n ointment in anal region 2 times a day specialy at night. Take. Tab. Felden 20mg

I needed to confirm these medicines with you again. I read felden tab is for pain related to arthritis, is it so? Can I take it for anal fissure? Secondly, the syrup that you prescribed is for constipation whereas I am not constipated at all because I am having ispaghol husk every night. Do I still use this syrup? bowel movements sre painful though; I feel like there is a sharp cut in there.

Please do reply. I cant wait to get rid of this pain

This problem is most likely due to Acute Anal Fissure which is caused by a scratch in the anal mucosa after having severe constipation. However, it can be due to other reasons also. You are already getting enough medication as symptomatic relief and I hope you will come out of this painful condition soon. You should avoid constipation by diet, medication etc. When possible you should see a surgeon for a proper treatment plan.

Whoch medicines ahould I take? Should I take caflam or felden? Felden I have read is for arthritis pain relief. I am so confused. And should i use laxative syrup?

Caflam is better. You can take one tab three times daily after meal. Do not take this medicine empty stomach because it can cause gastritis.
Yes; laxatives will help to soften stools and will protect this affected area from further damage. Another suitable practice is to apply ointment like TRONOLANE locally before passing stools. After passing stools hip baths with luke warm water ( with added pyodine) can give you some relief by giving local fomentation and for cleansing purpose also.

Almost all the fissure are cured by conservative means. some of them go into chronicity for which some more aggressive procedure is required.

Sorry to hear that. The cause of your problem may be either acute fissure or complicated piles but don't worry both are curable. You should visit any general surgeon for accurate diagnosis. Following medicines will relieve your symptoms.
1. Any painkiller. You can take Tab. Artifen 50 mg 3 times daily after meals.
2. Stool softener like syp duphalac 2 tablespoons at night.
3. 0.5 % GTN cream for local inside application 2 times daily. Excess may cause headache.
4. Tronolane ointment for local use better 15 minutes before passing stools.
5. Hop baths with luke warm water for local effect.
6. Dietary management with salad, vegetables etc to avoid constipation.
Hope it works till you visit a general surgeon for accurate diagnosis.

Ithink by now u have reviewed enough advice.Still I can revise & summarise it. 1.GTN Cream local appreciation with applicator twice daily.2. Lignocain Jelly twice daily apply as GTN with applicator also before or after passing stools to relieve pain. 3. Tablet caflam twice dsily after meal fir pain. Fruits, salad,vegetables, high fiber diet . You can also start Sitz bath which is sitting in luke warm water for 3/5 minutes in the morning & evening daily. This is most likely Acute Anal fissure will heal in a couple of weeks.Insha Allah.
