AoA, I have issue of Pilonidal Sinus since two years. Previously it used to discharge some pus and blood once in a month. Now it happens in once in weak ir 10 days.
Its a small opening on buttox. It inflammes a bit while pus accumulates and almost discharges on same day. Pus and blood is mild. Once small ammount of pus discharges it follows very small ammount of blood. Than this opening shows red meat inside. After discharge it reduces and flattens with skin in few days without any use if Antibiotic. What should be done sir? Is open wound surgery only option? How much area do yoy see involved by analyzing my description You are requested to guide me.

you need surgery as soon as possible
I think surgery will be the ultimate need may be open or laser is minimally invasive and least painful option with good results
Yes it's infected and it should be open surgery. If it's bad we usually don't close the margins with flap but leave it open to heal by secondary intention
karydakis flap is another option that's having promising results. good luck it's need operation
both laser and surgery can be done. laser is relatively a painless process. while after surgery u will need dressings for a month

This picture is after discharge and it almost flattens with skin. Discharge is v. minimal.
surgery is the best option, not open but a plastic surgery technique to close the wound primarily, I have 20 plus yrs experience doing it with very good results. but you have to come to lahore
Surgery is the only long term solution.

WS...you need to operate it prefferably open surgery. laser is option but not as effective as open.

Sir is there any specific scan or radiological test which could identify depth of this pilonidal sinus?

Surgery is the only option.
Sans have no relevance. Affected area needs excision.
