Assalamu Alaikum Doctor, I am currently around 24 weeks pregnant (my last menstrual period was on 15 September). In my recent anomaly scan, the doctor informed me that there is excess fluid in the baby’s head and the head size is larger than normal. They also mentioned a defect in the lower back. The doctor advised that there may be a serious risk to the baby’s life at the time of delivery and suggested termination of pregnancy. I am extremely worried and would like to seek your professional opinion before making any final decision. I would be grateful if you could please guide me on the following: Is a safe delivery possible through C-section in this condition? Are there treatment or surgical options after birth that could improve the baby’s condition? What level of risk is involved for the mother? Your baby is having multiple anomalies, although these are not life threatening but off course may cause serious issues to your baby after birth, so you need to take a opinion from peadriatric surgeon in your city that weather they can do their surgery or not or the facilities for the baby after birth are available or not. 2nd it’s only 24 weeks now the fluid in the baby head may increase, also there are other abnormalities that ultrasound may not show so usually we advise for termination of pregnancy as baby outcome usually seen is poor. But still this is your decision, if facilities are availing your city for your baby after birth then you should go for cesearian section

treatment decision dependson who much brain affected ,in case mild treatment with peadic neurosurgeon after deliver of baby ,only one ultrasound decision not made

do a second opinion DA ultrasound
take antenatal consultation with a neonatologist and ask about the issues if baby is born alive
hydrocephalus with another neural tube defect is not a good sign.
at best you should sit with your husband in consultations and make informed decision.
you may not see more than few hydrocephalus cases walking around. it is very challenging to take care of such babies and later when they become dependent adults.

Fluid in head will increase with time and press the brain matter, it can cause increased pressure, the skull may get bigger than body , baby will be in so much trouble , Ventriculo peritoneal shunts are put by paeda surgeons, they get small as body grows, dilated ventricale is more than 10 mm n brain matter is less than 10mm, it already shows that brain is compressed, babies with such problem can get fits , its hard for them to carry big head, also spina bifida will need surgery , it depends nerves are in that swelling or only membranes , and baby can have bowel n bladder control problem.. i suggest TOP is better for your family , and you have to decide soon , even at 24 wks baby may born alive when medicines are used , n letting your baby die in front of you without any help is so tough to see .. kindly take in person opinion from paeds surgeon , neurosurgeon n take decision , I have people taking decisions late n then the baby suffers in front of their eyes.. you have to arrange janaza and kafan dafan. Its really tough n heartbreaking ..
