Dear Doctor, I am seeking advice for my mother. She has been experiencing irregular menstrual cycles for the last few years. Her recent ultrasound shows a Uterine Fibroid on the posterior wall, measuring approximately 3.2 cm x 3.1 cm (noted as Type II). I would like to know: Given the size and location, is surgery mandatory, or can this be managed with medication? If surgery is required, how complex is the procedure for this size? Is a laparoscopic (minimally invasive) procedure an option in this case? I have attached the ultrasound scan and the initial prescription for your review. Looking forward to your expert guidance.

Yes laparoscopic surgery is possible. If she is experiencing heavy menstrual bleeding and her routine life is being disturbed then she needs surgery otherwise periods may become irregular near menopause.
at 48 with fitness she should have no problem to enter a surgery. But: What's the location of fibroid, is it projecting into the uterine cavity. once determined to be the reason of heavy menstrual loss ,she should have this surgery. minimal invasive surgery is an option and is done by a procedure of morcellation.v in Pakistan however, a laparotomy is mostly carried out

At this age surgery is not complicated, go for minimally invasive surgery only if the surgeon is expert in it , if he or she is not expert then they will cause more damage , open surgery or laparotomy experts are available everywhere, important decision is whether to remove only fibroid? Or hysterectomy? Hysterectomy with salpingoophorectomy ( tubes n ovaries) ? Depends on history n risk factors
