I always suffer from lack of concentration and have a weak memory can't memories things and can't remember things and forget things very easily I think may be I have dementia because it is affecting my daily routine and my job as well because I want to work but my brain always stuck in random thoughts plus no concentration

Dementia in such a young is possible but very rare.
Most probably this is pseudo dementia or brain fog. There are many causes of this problem. it requires detailed history and examination and maybe some tests to diagnose. my suggestion is you visit neurologist or atleast have a video consultation.
