Talk to Neuro Physician on Medication Discontinuation, Vertigo/ Anxiety

Asking For Mother, Female 50, Karachi

My mother has been prescribed 0.25 mg of Alp for 7 days and is supposed to discontinue its use by today since 7 days are complete.

Previously, she was diagnosed with BPPV (Benign Positional Vertigo) by one doc and Basilar Migraine by another. Since unko kafi arsay pehle 25 yrs ago se boht shadeed chakkar anay lagay thay, not normal ones, boht ziada uthte baithtay ya kharay hote huay.

For that she was prescribed Stugeron/ Serc and takes it whenever it happens until it goes away. Recently the frequency has increased as it is happening between every 4-5 months.

Lekin recently, she also has concerns regarding anxiety developing within her, due to which she insisted the doc to prescribe her some relieving/sukoon ki medicine - tou unhone Sert dedi for 15 days, but she couldn't take it due to fear of side effects. So she only took Alp for 7 days as told.

The issue is unclear right now. Whether she even has anxiety or not and whether treatment should be taken for it. Or if she only has Bppv and should continue taking only Stugeron until she's fine.

She is complaining of sleeping for 10+ hours after taking Alp and is unsure if stopping it right away after today is alright. Is it ok to stop immediately?

Please guide on the next steps. Whom to visit, what to do.

This does not sound like either of BPPV or Basillar Migraine. It could be however PPPD but need to speak to the patient to help.

Regarding the ALP, she can stop it.

If she has dizziness on standing up from a sitting position as you described then she doesn’t have BPPV but postural dizziness. To diagnose this check her blood pressure lying and standing. A standing drop of 15 or more is indicative of postural hypotension. Usual cause is blood pressure medication but there are other causes. Borderline cases are not unusual. BPPV is tested with Epleys manoeuvre and is best treated with certain exercises
