I need urgent guidance for my grandmother (79 years old). Last night she suddenly developed weakness on the left side of her body, facial drooping, speech difficulty, and she is unable to eat or drink. She is extremely weak. We did not recognize the symptoms immediately. At the moment, it is very difficult for us to bring her to a hospital. We would like to request an urgent video consultation or guidance on what immediate steps we should take at home, and whether a home visit or emergency care is necessary. Please let us know how to proceed as soon as possible.

Having said that, she needs a CT scan as the bare minimum to determine whether it is an ischemic or hemorrhagic stroke. This will guide further management.

Your aunt has had a stroke .,A major blood vessel in the right side of her brain is blocked due to a clot obstructing it. It would be best if you can get a CT scan of her brain. Make an appointment urgently to see me for online advice. The main problem at this stage is difficulty in swallowing. Any attempt at feeding her with a spoon as is commonly done by the family can result in aspiration pneumonia which can be fatal. So if she can swallow sit her up to feed her. Don’t try to treat to feed her in lying position. If she has an infarct then you will need to start her on an antiplatelet most importantly Clopidogrel to stop the clotting process. At this stage her BP would be high. Don’t attempt to bring it down to normal level. Keep her hydrated. The best way ( if she can’t swallow is through a nasogastric or NG tube.
