Talk to Neuro Physician on Weakness On Face Cheeks

for 3 years I have been dealing with this I have weakness numbnes of my cheeks which makes my face visibly hollow sunknen and it's not in mind because other people also point this out some days it's get fuller but most days remains worse along with numbness weakness what can be the issue of this sunknenes on and off I'm already low of b12 and vit d but no Improvement with IM I have attach pic below
