Asking for Mother, Female, 55 years old, Sadikabad

AOA Sir/Ma'am I hope you all are fine and having a great life. I am here to consult about my mom's health. She has been ill for almost 8 years now. She is still not mentally stable. " years back, we started her treatment just by diagnosing her condition to Dr. as she is not willing to leave the house. this is nor 4 years she hasn,t stepped out of the home. Risperidone, Kempro, Olanzia ans qusel were her prescriptions time by time. She literally got cured by having the combo of these medications at different stages but became very weak and sleepy so we stopped her medication. But now again the problem is elevated. One problem which is very important didn't get cured her social life. She isn't willing to step out of the house still. See if you can help me accordingly. She has no medical reports as per the problem mentioned above!

i am more interested in her complaints and history for which medications were started.i am interested to know her age and weight and physical status.i am interested to know i she diabetes or hypertention or any other medical problem.what is her memory status

i am sorry for the distress you are in. since last so many years.Bipolar
patients need great family support ofcourse.
please tell me if her illness been episoidic?
since how long she remained well during these episode,like symptom free
period?
what are her current symptoms,
please tell about her mood,is she depressed,or elated.
and what about her sleep? social functioning ?

Thank you so much dr. Iqra for showing interest in her case. Means a lot.
She remained completely fine for about two years except for the socialism phobia.
Current symptoms clearly showing that she is having hallucination problem or bipolar.
She is having good sleep I guess because of these medicines. And social functioning is nill.
All she says that she is having the glimpses of her sister and mother teasing her all the time. We have started again her treatment by giving her kempro and qusel (150 mg) two times a day. But she hasn't responded to this.
See if you can help me in this case, I can't bring her to you as she is severely ill and can't let us do this. Share your expertise accordingly. I'll be honored.

ASA, this case required expert opinion, aim would be to take detailed history which would assess the symptom, severity, duration and course of illness and impact on life. as for as current treatment is concert which need to be review that patient will not suffer the side effects of the medication which will help to assess the response of the treatment..... thanks

Here it is the brief history of her, explaining you her history, nearly 8 years ago she suddenly started acting weirdly as per my memory, As we were kids at that time so we couldn't do well to her and moreover my father got brain hemorrhage at the same time so we couldn't stand by her condition. Nearly 4 years back we started taking action against her hallucinating acts by just narrating her nature to Dr. luckily, his prescription got us some relieved and she got better but the only problem that was intact and still is her socialism phobia, she hasn't stepped out of the home till 4 years and we 've tried many times but failed. almost years back I guess we stopped her medication because she was getting weaker and more sleepy. but I guess we were wrong because that disease hit her again brutally. We have started again her treatment by giving her kempro and qusel (150 mg) two times a day. But she hasn't responded to this. All she says that she is having the glimpses of her sister and mother teasing her all the time. See if you can help me in this case, I can't bring her to you as she is severely ill and can't let us do this. Share your expertise accordingly. I'll be honored.

Her age is 55 and weight is nearly about 40.

It is very characteristic of mental illnesses in general and particularly
bipolar illness that patient frequently stop medications as soon as they
are better. It is one of those illnesses who get recovered to almost the
previous state of health. So usually the target is to maintain some kind of
minimum medications as warranted by the patient health and past response.

Main issue now is not the DIAGNOSIS, nor the TREATMENT rather what we call
as COMPLIANCE.

Ideally when she was better, some kind of therapeutic intervention could
have been employed to make a sense of her illness and the need to take care
of medications for a certain amount of time.

I think this time, work with any given psychiatrist in your nearest
location, and make a short term and a longterm planning .

At the end, its very stressful for the caregivers. Psychiatrists here
understand fully your plight. Don t keep yourself from asking for any
guidance and help.

thanks for detailed account of the patient but still this case need to be assessed which very important for which patient should be seen by a psychiatrist and history is more towards schizophrenia for which patient should be on medication and along with that family has to be educated about the illness and illness management which is not possible in this way, she should be assess if she is the resistant case in which she may be put on the medication therapy according to her underlying illness, duration and severity of symptoms and choice of drugs would be the one which would not have sedative properties, if she required sedative psychoanalytic that must be in the night time, so it is advised to visit a psychiatrist or get her hospitalized that she could me managed properly not only from psychiatric perspective but physical condition also........ thanks

Aoa. Thank you for choosing this platfrum for the mentioned help, for this pertinent issue, as a Psychiatrist it is very necessary to assess the patient along with her current mental state examination, to review the diagnosis and her prior compliance to the medications?
does she has any associated medical ilness like Hypertenison, Diabetes or Hypothyroidism? and asking about the family dynamics and support in the family?

I would suggest adding of antidepressant along with addition of Tab lamotrigine 100mg BD along wit Tab Risperidone and tab kempro as prescribed previously.
