Asking For Brother, Male 43, Islamabad

-Restless -Fixated on ordinary things -Castle in thin air -Delusional -Likes boasting -No concern for hygiene -Soft spoken but no regard to logic -Lies or deflects direct questions -Severe insomnia -Use to see and hear unseen things -Wants to boast but otherwise unconcerned of society -Doesn't like to talk about religion or any kind of morality -Somewhat anti god sentiments -Thrice divorced due to family pressure as well as unable to manage marriage -Fearful about sexual health -Use to take drugs and sleeping pills - no regard or understanding of moral values.

Don't admit that he has any depression or problems. Used to talk about hallucinations but now categorically refuses even past stories of hallucinations.

Talkative but gets over excited over minor things of liking. Appears very obedient and passionate at least till not confronted with opposition or asked directly. Not aggressive but too repetitive and stubborn.

Used to talk about Djinn, fairies and black magic. Now we

Then it is unlikely to be truely psychotic if he could work with these symptoms … and may be a deep rooted personality disorder. But only a thorough discussion ( with a closest relative who can give specific history , in case of patient not being co - operative ) will give more clear picture . I an happy to help by a video / phone consultation of 30-45 mint . Let me know if you want to book one . Best wishes

Patient lacks insight into his illness which happens mostly in psychotic illnesses. kindly book appointment for a thorough assessment and treatment plan.
if Patient is unwilling any family member can consult to understand different treatment options available

I am obliged by candid replies by all the doctors.

I am aware already that the patient needs complete assessment in person but he is not willing and nor does he have any insight that he is ill psychologically.

secondly, the reason for my question was to get a professional opinion on whether these symptoms appear to be any indication of illness or can it be a mere careless attitude.. especially if the person did 24 years service in the armed forces.

please do give me an opinion on how to get him to see some doctor also.

Seems like a chronic case of a psychotic illness might even need admission for even assessment and proper treatment plan due guarded nature/lack of insight.
