Talk to Rheumatologist on Severe Foot Pain

Aoa. The patient has history of a heavy object falling on his foot 10 years back. It got better with time but now he again feels severe pain. He visited a doctor recently and has been taking the prescribed medication but hasn't felt any relief. The reports and x-ray is attached. Do we need to get a seco.d opinion or stick with the current treatment?Please guide. Also it would be kind if you could elaborate what the x-ray shows. Thanks
