About Frozen Shoulder

Frozen Shoulder, also known as adhesive capsulitis, is a condition that causes stiffness, pain, and limited movement in the shoulder joint. It develops when the connective tissue around the shoulder becomes thick, tight, and inflamed. This restricts the joint’s ability to move smoothly.

The condition usually progresses slowly in three stages. These include freezing, frozen, and thawing. This condition can take several months or even years to fully resolve. Some people recover with simple exercises and non-surgical treatments. However, others may need physical therapy or medical intervention to regain mobility.

Frozen Shoulder Symptoms

Frozen shoulder symptoms usually develop gradually. Contact a doctor if you notice these signs:

  • Persistent shoulder pain
  • Increasing stiffness in the joint
  • Difficulty lifting the arm
  • Limited range of motion
  • Pain worsening at night
  • Difficulty reaching behind the back
  • Shoulder feels “stuck” during movement

Contact your doctor immediately if you notice:

  • Severe and sudden shoulder stiffness
  • Inability to move the arm at all
  • Weakness along with pain
  • Swelling, redness, or signs of infection
  • Worsening pain despite rest

Frozen Shoulder Causes

Frozen shoulder develops due to inflammation and thickening of the shoulder capsule. Common causes include:

  • Shoulder immobility after injury or surgery
  • Diabetes (one of the strongest risk factors)
  • Hormonal changes
  • Long-term shoulder inflammation
  • Autoimmune or inflammatory disorders
  • Thyroid problems (underactive or overactive thyroid)
  • Stroke
  • Shoulder arthritis
  • Prolonged use of arm sling
  • Scar tissue formation around the joint

Frozen Shoulder Risk Factors

Certain factors increase the risk of developing frozen shoulder, such as:

  • Age above 40
  • Diabetes or prediabetes
  • Shoulder injury or surgery
  • Prolonged immobilization
  • Thyroid disorders
  • Heart disease
  • Long-term inflammation
  • Arthritis
  • Parkinson’s disease
  • Previous frozen shoulder in the other arm
  • Autoimmune conditions

Frozen Shoulder Prevention

Although frozen shoulder cannot always be prevented, these measures may reduce risk:

  • Regular shoulder mobility exercises
  • Avoid keeping the shoulder or affected area completely still after injury or surgery
  • Managing diabetes and thyroid disorders effectively
  • Gentle stretching after long periods of inactivity
  • Treating shoulder pain promptly
  • Maintaining good posture
  • Avoiding repetitive strain on the shoulder
  • Early physiotherapy after surgery or injury

Frozen Shoulder Types

Frozen shoulder progresses in three well-defined stages:

Freezing Stage:

In this stage, movement slowly starts to hurt, and stiffness gets worse. The pain increases with time, especially when you use your shoulder.

Frozen Stage:

In this condition, the pain may get better, but the stiffness becomes worse, and the shoulder cannot move properly.

Thawing Stage:

In this stage, movement begins to improve gradually, and the shoulder slowly regains normal function. However, full recovery can take several months.

Some individuals may experience a combination of symptoms, depending on the underlying cause and severity.

Frozen Shoulder Diagnosis

When someone presents with symptoms of frozen shoulder, doctors use the following steps:

  • Physical examination to assess movement
  • Evaluation of pain and stiffness levels
  • Review of medical history and past injuries
  • X-ray to rule out arthritis or fractures
  • MRI or ultrasound to detect inflammation
  • Assessment of muscle strength
  • Passive and active range-of-motion testing
  • Blood tests if underlying conditions are suspected
  • Shoulder joint evaluation for thickening or swelling

Frozen Shoulder Treatment

Frozen shoulder treatment focuses on reducing pain and restoring movement. Management varies depending on the stage. Different treatment approaches include:

Therapies:

  • Physical therapy to improve joint flexibility
  • Stretching and strengthening exercises
  • Heat therapy before stretching
  • Manual therapy for joint mobility
  • Home-based exercise programs

Medications:

  • Pain relievers such as NSAIDs
  • Anti-inflammatory medications
  • Corticosteroid injections to reduce swelling
  • Muscle relaxants for pain relief

Medical and Surgical Interventions:

  • Hydrodilatation to expand the joint capsule
  • Shoulder manipulation under anesthesia
  • Arthroscopic surgery to release tight tissues
  • Treatment of underlying conditions like diabetes or thyroid issues

Supportive Services:

  • Guidance on posture and daily movements
  • Assistive devices if needed
  • Lifestyle modifications to prevent recurrence
  • Regular follow-up with orthopedic and physiotherapy teams

Frozen Shoulder Complications

Without proper care, frozen shoulder may lead to complications such as:

  • Long-term stiffness
  • Persistent pain
  • Difficulty performing daily tasks
  • Sleep disturbances due to pain
  • Reduced shoulder strength
  • Increased risk of falls or injuries
  • Muscle imbalance
  • Adhesion formation in the joint
  • Recurrence in the other shoulder
  • Psychological stress due to limited mobility

The severity of complications depends on timely treatment and consistency with exercises.
