Talk to Gastroenterologist on Perianal Fistula

Iam facing this perianal fistula problem form last 30 days , general Doctor give me different antibiotics to but they are effective for just week. It's occur again for the first tym its was at 6o clock With hard gulit ,but no blood and no open mark just hard area near anal now it's making pain again at 7o clock so kindly help me what can i do to get rid from that problem.
