Treatment History & Response:
1. Prednisolone (Steroids):
On steroids continuously since diagnosis.
High doses (50–60 mg/day) were effective in controlling symptoms.
Low doses (<30 mg/day) are ineffective, and symptoms return immediately.
2. Infliximab (IV Infusion):
Received 3 induction infusions.
No significant clinical improvement observed.
3. Mesalazine (1g twice daily) + Steroids:
Tried combination therapy.
No improvement in symptoms.
4. Upadacitinib (JAK inhibitor):
Trial started.
No clinical response.
5. Current Plan:
My doctors have now decided to start Mirikizumab (IL-23 inhibitor) as the next line of biological therapy.
I am awaiting my first dose.

you must continue steroids until mirikizumab start it's action usually in 2-4 weeks. after that if it not works,You may switch to vedolizumab or tofacitinib. Hope for the Best. InshaAllah you'll be All good.
