Treatment History & Response:

1. Prednisolone (Steroids):

On steroids continuously since diagnosis.

High doses (50–60 mg/day) were effective in controlling symptoms.

Low doses (<30 mg/day) are ineffective, and symptoms return immediately.

2. Infliximab (IV Infusion):

Received 3 induction infusions.

No significant clinical improvement observed.

3. Mesalazine (1g twice daily) + Steroids:

Tried combination therapy.

No improvement in symptoms.

4. Upadacitinib (JAK inhibitor):

Trial started.

No clinical response.

5. Current Plan:

My doctors have now decided to start Mirikizumab (IL-23 inhibitor) as the next line of biological therapy.

I am awaiting my first dose.

Current Condition & Ongoing Symptoms:

Persistent active disease with high symptom burden.

It's really a painful condition. I understand your situation . But i believe You'll get rid of this disease. please follow Your doctor advice as they're doing best to manage your condition.
you must continue steroids until mirikizumab start it's action usually in 2-4 weeks. after that if it not works,You may switch to vedolizumab or tofacitinib. Hope for the Best. InshaAllah you'll be All good.
