Dear Doctor my two year old baby boy has been suffering with this for a long time ,it seems very itchy and uncomfortable please advise the reason and remedies with proper treatment picture attached

This is most likely atopic dermatitis also called eczema. It is common in children. It will increase by dryness, scratching and sweating. Please check if there is some specific allergy to any clothing or soap.
Management:
Avoid scratching
Use normal temperature water and avoid hot water for washing
Use baby soap instead of normal soap
Use soft cotton clothes
Use good moisturiser after bath
Treatment:
Hydrocortisone 1% twice daily for 7 days
Syrup Loratadine 5ml at night (if too much itching)
CERAVE ECZEMA CREAM Twice daily and after bath
Inform after one week

Mostly like diagnosis is ringworm infection. It is a fungal infection.
Treatment :
Syp. Rigix (2.5 ml i.e half teaspoon before sleep)
Cream Candid (apply on lesion twice daily)

If lesions are multiple and occurring repeatedly, then add Syp. Grivosin FP (once daily after any fatty meal like milk, butter, ghee) for 1 week. Might be needed to given for another week depending on condition of infection.
