Talk to General Physician on Pain In Body,fatigue, Sleep Issues, Headaches, Jaw And Shoulder Stiffness

I want to get myself checked but I live far away so can you advice me blood tests I can bring beforehand to make it easy. I have extreme body pains and feel tired all the time. I wakeup every half hour in sleep and no matter how long I stay in bed, i am tired. my shoulders and neck and jaw are always stiff but mostly when I have headaches. everytime I lay on bed, my legs go into extreme pain. pain increases at night and in A.C. I also have some really bad stomach issues including constipation with it. my vit d levels were low few years back but since then I have been taking supplements and Vit d injections regularly however my muscle fatigue hasn't relieved a bit. my hb is 11 for the reference. what other tests can I bring along ?
