Talk to General Physician on Pain In Left Arm, Left Shoulder, Upper Central Back

The pain was felt 2.5 years ago too, but it was taken lightly. Now the pain is consistently being felt from past 4-5 days. It started with pain in left arm (bicep & upper elbow area) & then in left shoulder’s & upper back(right behind heart). Ghabrahat si hoti hai lait kr so this feeling is not letting her sleep, maybe which is why she is feeling pain in left side of head (due to sleeplessness/ tension). It feels like symptoms of Heart Attack / Angina. Please tell what to do? A doctor recommended the following: (see the attached pic)
