Patient is my mother named Safia parveen 51 year of age female muslim marriied house wife by profession k/c of *lumber rediculopathy* at L4.L5,S1 and *scalp psoriasis* Chief complaint=Severe generalized weakness HOPC=According to the statement of my mother she feels pain in head and back and severe generalised weakness for last 1 year she was admitted in Zaiuddin hospital where she was diagnosed with lumber rediculopathy For which she now take medication Now the problem for weakness has not resolved Even she take Oslia 200,000 IU Personal hx=normal appetite,sleep distrubed,normal bowel and bladder,no any addiction. Past medical hx=she had hx of renal stones for which she had been treated In this regards kindly give proper management for weakness
